Cardizem LA, a diltiazem hydrochloride extended release tablet, is used alone or with other antihypertensive medications, to treat hypertension. Cardizm LA is also used for the management of chronic stable angina.
Cardizem LA Tablets are for oral administration and are formulated as a one-a-day extended release tablet containing 120 mg, 180mg, 240mg, 300mg, 360mg, or 420mg of diltiazem hydrochloride. The tablets are white and capsule shaped with “B” deposed on one side and the diltiazem content (mg) on the other side and should be stored at 77 °F with excursions permitted to 59-86°F. Excessive humidity an temperatures above 86°F should be avoided.

Diltiazem hydrochloride is a crystalline, white to off-white powder that is a calcium ion cellular influx inhibitor with a bitter taste. It is soluble in water methanol and chloroform and has a molecular weight of 450.99. Diltiazem hydrochloride is chemically 1,5-benzothiazepin-4(5H)one,3-(acetyloxy)-5-[2- (dimethylamino)ethyl]-2, 3-dihydro-2-(4-methoxyphenyl)-, monohydrochloride, (+)-cis-.
If a patient is proscribed on diltiazem alone or in combination with other medications they may be switched to the nearest equivalent total daily dose of Cardizem LA Tablets once-a-day. Higher dosed may be required in some patients but those patients should be closely monitored. As clinically warranted, Subsequent titration to higher or lower doses may be necessary and should be initiated. The tablets should be crushed or swallowed whole. As doses increase the incidence of side effects may also increase.

For treatment of hypertension with Cardizem LA the dosage should be adjusted by titration to individual patients need. Reasonable starting doses for monotherapy are 180mg to 240mg once a day but some patients may respond to even lower doses. The maximum effect is usually obtained by 14 days of chronic therapy. Tablets should be administered around the same time once each day. 540mg daily is the maximum dosage that may be titrated.
For treatment of angina, dosage should be individualized based on response. The initial dose of Cardizem LA should be 180 mg once daily and may be increases if adequate response is obtained at intervals of 7-14 days. Doses above 360mg confer no additional benefit. Dose should be administered once daily in the morning or in the afternoon.
Appropriate supportive measures should be employed in addition to gastrointestinal decontamination in the event of an overdose. Diltiazem should not be used in patients with sick sinus syndrome except in the presence of a functioning ventricular pacemaker, second- or third-degree AV block except in the company of a functioning ventricular pacemaker, hypotension (less than 90 mm Hg systolic), patients who have established hypersensitivity to the drug, and patients with acute myocardial infarction and pulmonary congestion documented by x-ray on admission.
